Alere’s CareAlerts program proactively screens for care gaps across each client’s population.The goal of the service is to help physicians and participants become more aware of potential care gaps,such as poor compliance or missed preventive screenings and to close those gaps – thereby reducing potential risks.In addition,the service identifies new cases,tracks changes in therapy and determines if any follow-up is necessary.
Alere’s CareAlerts program provides physicians directed alerts based on medical and pharmacy claims.Alere evaluates administrative claims data (medical and prescription),lab result data,participant eligibility files,participant benefit information,and provider information,to identify potential discrepancies between actual care (either acute or chronic) received by an individual and evidence-based guidelines or other best practices (e.g.,avoidance of potentially dangerous drug-drug and drug-condition combinations).We use ICD-9 codes to identify conditions; CPT -4,HCPCS,and ICD-9P codes to identify tests,labs,and procedures; NDC codes to identify drugs; and LOINC codes to identify lab results.The program also includes participant-facing alerts that encourage participants to reach out to their healthcare provider as well as suggestions on ways to manage their health.Specific participant benefits of the program include:increased compliance with prescribed therapies and clinical guidelines; improved quality of life and overall health; and reduced absenteeism rates and healthcare costs.To assist participants in understanding the program and related benefits,Alere provides a Frequently Asked Questions document.
